What Is TMJ Disorder?
The temporomandibular joint (TMJ) is the hinge that connects your jaw to your skull. It is one of the most complex joints in the body, allowing your jaw to move up and down, side to side, and forward and back. When this joint — or the surrounding muscles — are out of balance, the result is TMJ disorder (also called TMD).
Common TMJ symptoms include:
- Jaw pain, soreness, or stiffness — especially in the morning
- Clicking, popping, or grinding sounds when opening or closing your mouth
- Frequent headaches or migraines, often felt at the temples
- Neck and shoulder pain or stiffness
- Ear pain, ringing in the ears (tinnitus), or a feeling of fullness in the ear
- Difficulty chewing, biting, or fully opening your mouth
- Teeth grinding (bruxism) or clenching, often during sleep
- Worn, chipped, or sensitive teeth
The Neuromuscular Approach: Treating the Root Cause
Traditional dentistry often focuses on the teeth and bones. Neuromuscular dentistry goes deeper — it looks at the entire system: the teeth, jaw joints, muscles, and nerves working together. The goal is to find the position where your jaw is most relaxed and balanced, and then stabilize your bite in that ideal position.
How is this different from conventional TMJ treatment?
Many patients come to us after years of being told their pain is “stress-related” or after receiving only temporary relief from treatments that did not address the underlying bite imbalance. Neuromuscular dentistry uses advanced technology to objectively measure how your jaw functions, removing the guesswork.
How We Diagnose Your Condition
We begin with a detailed, comprehensive exam that carefully evaluates both the jaw joint and the surrounding muscles to identify any areas of imbalance or dysfunction. We also perform a digital scan to precisely analyze your bite, giving us an accurate picture of how your teeth come together and how that may be affecting your overall comfort and health.
Our Treatment Options
- TENS Therapy (Transcutaneous Electrical Neural Stimulation)
Treatment often begins with TENS therapy — a gentle, painless electrical stimulation that relaxes the jaw muscles. This allows the jaw to settle into its natural resting position, free from the muscle tension that has built up over time. Many patients feel immediate relief.
- Orthotics (Neuromuscular Splints)
Once we identify your ideal jaw position, we create a custom orthotic — a precision-fitted appliance worn over your teeth that holds your jaw in the correct position. Unlike generic night guards, a neuromuscular orthotic is calibrated specifically to your measurements. Most patients experience significant pain reduction within weeks.
- Long-Term Bite Stabilization
After symptoms have resolved, we discuss permanent options to maintain your corrected bite. Depending on your needs, this may include carefully reshaping certain teeth, dental restorations such as crowns or veneers, or orthodontic treatment — all aimed at keeping your jaw in its healthy, balanced position.
The TMJ – Sleep Apnea Connection
Many patients are surprised to learn that TMJ disorder and sleep apnea are closely linked. A poorly positioned jaw can obstruct the airway during sleep, contributing to snoring and obstructive sleep apnea (OSA). Conversely, sleep apnea can worsen nighttime clenching and grinding, further stressing the jaw joints.
At Palo Alto Dental Center, we can screen you for sleep-disordered breathing right in our office. We have a sleep testing device available for patients to borrow at home — giving you a convenient, comfortable alternative to an overnight sleep lab. For patients diagnosed with mild to moderate sleep apnea, a custom oral appliance can reposition the jaw to keep the airway open, offering a CPAP-free solution many patients prefer.
